COLLECTION, USE AND DISCLOSURE OF PERSONAL INFORMATION
CONSENT FORM - CREDIT BUREAU REPORTING
1. BACKGROUND
AvenaCredit provides credit-building services to consumers in Canada, including tradeline reporting, rent payment reporting, utility payment reporting, and backdated rent history reporting (collectively, the "Services"). In order to deliver these Services, AvenaCredit collects, uses, and discloses your personally identifiable information ("Personal Information") as described in this consent form.
AvenaCredit reports your account activity and payment performance to Equifax Canada and/or any other applicable consumer reporting agency (collectively, the "Credit Bureau(s)") so that your credit file reflects your participation in the Services.
2. PERSONAL INFORMATION COLLECTED
AvenaCredit may collect the following Personal Information in connection with the Services:
Full legal name (first and last name)
Date of birth
Current and previous residential addresses
Email address and telephone number
Social Insurance Number (SIN), where required for credit bureau matching purposes
Payment history, including on-time, late, and missed payments
Tradeline details, including credit limit, balance, and account status
Rent payment amounts and tenancy details (landlord name, lease dates, rental address)
Utility account details and payment history
Bank account or payment card information for subscription billing
3. PURPOSE OF COLLECTION AND USE
Your Personal Information is collected, used, and disclosed for the following purposes:
To establish and maintain your AvenaCredit account and deliver the Services you have subscribed to;
To report your account activity, tradeline data, and payment performance to the Credit Bureau(s), including on-time payments, late payments, missed payments, account balances, credit limits, and account status;
To report your current and/or historical rent and utility payment data to the Credit Bureau(s);
To verify your identity for credit bureau matching and fraud prevention;
To process your subscription payments and manage billing;
To communicate with you regarding your account, payment reminders, and service updates;
To comply with applicable laws, regulations, and legal obligations.
4. CONSENT TO REPORT TO CREDIT BUREAU(S)
By enrolling in AvenaCredit’s Services, I authorize AvenaCredit to collect, use, and disclose my Personal Information - including my name, address, date of birth, Social Insurance Number (where provided), and payment history - to Equifax Canada and any other applicable consumer reporting agency for the purpose of reporting my account activity and payment performance.
I understand and acknowledge that:
My on-time payments, late payments, and missed payments may be reported and reflected on my consumer credit file;
This reporting may positively or negatively affect my credit score depending on my payment behaviour;
Tradeline information reported to the Credit Bureau(s) will include the credit limit, outstanding balance, payment status, and account standing associated with my AvenaCredit account;
Rent and utility payment data, including any backdated history I authorize, will be reported as tradeline(s) on my credit file;
Information reported to the Credit Bureau(s) becomes part of my consumer credit file and may be accessed by third parties (including lenders, landlords, insurers, and employers) in accordance with applicable law;
AvenaCredit cannot retract, amend, or remove data that has already been reported to the Credit Bureau(s), except as required by law or to correct inaccuracies.
5. CONFIDENTIALITY AND SECURITY
AvenaCredit is committed to protecting the confidentiality of your Personal Information through appropriate security safeguards. Your Personal Information will be stored in AvenaCredit’s secure operating systems and will only be accessed by authorized personnel for the purposes described in this consent form. AvenaCredit maintains administrative, technical, and physical measures designed to protect your Personal Information against unauthorized access, disclosure, alteration, or destruction.
6. DISCLOSURE TO THIRD PARTIES
AvenaCredit may share your Personal Information with the Credit Bureau(s), payment processors, and service providers who assist in delivering the Services. In such cases, AvenaCredit will ensure that appropriate contractual safeguards are in place to protect your Personal Information. Third parties will only have access to your Personal Information to the extent necessary to perform their functions and are required to maintain its confidentiality.
In some cases, where required by law or competent authority, AvenaCredit may be required to disclose your Personal Information.
7. RETENTION
Your Personal Information will be retained for the duration of your subscription and for a reasonable period thereafter to complete any final reporting obligations and to comply with applicable legal and regulatory requirements. Your Personal Information will be securely destroyed upon the earliest of:
Your written request to close your AvenaCredit account;
The withdrawal of your consent, subject to any outstanding reporting obligations;
The expiry of any applicable legal retention period.
8. WITHDRAWAL OF CONSENT
You may withdraw your consent at any time by cancelling your subscription and notifying AvenaCredit in writing at the contact information below. Please note that withdrawing your consent will result in the termination of the Services and that any data already reported to the Credit Bureau(s) prior to the effective date of withdrawal cannot be retracted by AvenaCredit. Your tradeline(s) will be reported as closed to the Credit Bureau(s) upon cancellation.
9. RIGHT OF ACCESS AND CORRECTION
You have the right to request access to the Personal Information that AvenaCredit holds about you and to request corrections if any information is inaccurate or incomplete. AvenaCredit will respond to your request for access, correction, or information within 30 days following receipt of the request.

10. APPLICABLE LEGISLATION
This consent form is governed by the Personal Information Protection and Electronic Documents Act (S.C. 2000, c. 5), the Act Respecting the Protection of Personal Information in the Private Sector (RLRQ, c. P-39.1), and any other applicable federal or provincial privacy legislation, depending on the relevant jurisdiction.
11. CONSENT
By checking the consent box during the AvenaCredit sign-up process, you confirm that you have read, understood, and agree to the terms of this consent form, including the collection, use, disclosure, retention, and reporting of your Personal Information to the Credit Bureau(s) as described herein.
